April 16th is National Advance Care Planning Day, a day for you to think about and share your wishes for future health care with your Substitute Decision Maker—someone who can speak for you if you can’t speak for yourself. But don’t forget to talk to others in your community too—your partner, your family, your friends… they can help support you and your Substitute Decision Maker.
Advance Care Planning is a process of reflecting on your values and wishes, and letting others know what kind of health and personal care you would want if you were unable to speak for yourself.
