CKFHT employees and CKFHT contractors, may use this Whistleblower process to report suspicion or proof of wrongdoing, defined as any kind of activity that could be deemed illegal, fraudulent, gross waste of funds, or misuse of CKFHT assets. Employees and contractors of CKFHT may use the Whistleblower process without fear of reprisal under the CKFHT’s whistleblower policy. Please review the CKFHT Whistleblower policy outlining what may be submitted through this process before submitting a complaint or concern.
Submitting a Complaint
There are two ways to report suspicion or proof of wrongdoing:
- Anonymous Submission – Submit a secure Whistleblower On-Line Suspicion form and indicate on the form that the report is being provided ANONYMOUSLY.
- Non-Anonymous Submission – Send an Email to whistleblower@ckfht.ca by email outlining the details of your concern. You may be contacted for additional information if required.
Note: In the interest of ensuring accountability and responsibility in reporting, anonymous complaints will be investigated to the extent possible, as anonymity creates limitations to the investigation.
