Memory loss, as a medical disorder, involves the inability to recall information on a regular basis. A memory disorder is disabling and consistent, interfering with work or activities of daily living.

The Cognitive Screening Program is designed to ensure that all CKFHT patients have access to appropriate cognitive screening and to ensure that all CKFHT patients identified with dementia are assessed for driving.

Ask your Health Care Provider for an assessment if you are concerned about your memory or that of a loved one. A referral for more extensive testing can be arranged.
