A Certified Respiratory Educator/Heart Failure Educator/Case Manager, in collaboration with  your Physician or Nurse Practitioner, will work with you to confirm a diagnosis with Spirometry testing and optimize medications to evidence based guidelines for Asthma, COPD (Chronic Obstructive Pulmonary Disease) and Heart Failure & Atrial Fibrillation.

We will also:

• Improve your knowledge
• Support medication adherence
• Ensure effective delivery of medication
• Encourage increased and regular activity and exercise
• Support nutritional needs, including salt and fluid intake
• Support for anxiety & other mental health challenges
• Identify & support management of co-morbidities
• Support smoking cessation
• Offer self management training
• Support your confidence for self management
• Create an action plan
