A Nurse Educator will provide Individual Education sessions focusing on:

• Individual Cardiac Procedures
• CAD Etiology
• Healthy Targets (e.g. BP, pulse, Cholesterol)
• Healthy Diet
• Medications
• Exercise Do and Don’ts

This Program is for any patients who were discharged from hospital after receiving care for any post cardiac event.
