Chronic Disease Self –Management

Weekly 2.5 hour group classes for 6 weeks with focus on strategies to manage chronic disease:

  • Management of Stress, Pain, Fatigue, Emotions
  • Healthy eating and exercise
  • Communicating with health team
  • Medication Use
  • Relaxation and Breathing Techniques
  • Use of Action Plans

Chronic Pain Self –Management

Weekly 2.5 hour group classes for 6 weeks with focus on strategies to manage chronic pain:

  • Management of Stress, Anger, Fatigue, Depression, Frustration related to Chronic Pain
  • Exercise for maintaining and improving strength, balance and flexibility and endurance
  • Pain medications, treatment and relaxation techniques
