The Medication Therapy Management Program provides CKFHT patients with assistance to manage their medications. Specific support for those persons:

• on Warfarin (Coumadin)
• Medication review
• Deprescribing (the process of intentionally stopping a medication or reducing its dose to improve the person’s health or reduce the risk of adverse side effects)

Working with your Health Care Provider, a Pharmacist and Registered Nurses will meet with you to ensure that you are on the correct dosage and medications for your needs.
